Step 1: Complete Enrollment Forms
- Enrollment Application (Includes basic student and family information)
- Parent Contract & Financial Agreement
- Medical Information & Immunization Records Form
- Authorization for Emergency Medical Care & Transportation
- Emergency Contact & Release
- Media Release
- Physicians Report for Child Care Center
- Eating, Sleeping, and Toilet Habits
- Development Form
Step 2: Provide Necessary Documents
- Copy of Birth Certificate
- Copy of Immunization Records (or religious exemption form, if applicable)
- Copy of Health Examination/Physical Form (dated within the last year)
Step 3: Review and Acknowledge Policies
- Read and sign the Parent Handbook Acknowledgment Form
- Read and sign the Faith Statement Agreement
- Read and sign the Discipline Policy Agreement
- Read and sign the Tuition & Financial Agreement
Note: These Acknowledgements will be provided after the school tour.
Step 4: Schedule and Prepare for School
- Pay the Registration Fee (Non-refundable)
- Purchase necessary school supplies (provided during tour)
- Schedule a tour (click here)
- Attend the Parent Orientation (First week of school)
